1. Acknowledgment of Risk

I understand that participation at Ryze Martial Arts & Fitness Academy may include Brazilian Jiu-Jitsu, Judo, Muay Thai, martial arts training, fitness exercises, stretching, drills, partner work, sparring, takedowns, grappling, striking, self-defense instruction, open mat, weight room use, sauna use, academy events, and other physical activities.

I understand that martial arts, fitness training, weight training, sauna use, and contact sports involve risk. These risks may include, but are not limited to, bruises, cuts, sprains, strains, muscle soreness, falls, accidental contact, heat-related illness, dehydration, or more serious injury.

I acknowledge that injuries may occur even when safety rules are followed and instructors are supervising.

By signing this agreement, I voluntarily choose to participate in training, classes, events, and activities at Ryze Martial Arts & Fitness Academy.

2. Release of Liability

I agree to release and hold harmless Ryze Martial Arts & Fitness Academy, its owners, instructors, coaches, staff, volunteers, representatives, affiliates, and facility partners from liability for injuries, accidents, losses, damages, illness, or personal property loss that may occur during participation in classes, training, open mat, academy events, weight room use, sauna use, or any academy-related activity, except in cases of gross negligence or intentional misconduct.

I understand that I am responsible for my own safety, conduct, and physical condition while participating.

3. Health and Medical Responsibility

I confirm that I am physically able to participate in martial arts, fitness training, and other academy activities.

I understand that it is my responsibility to consult a physician before participating if I have any medical condition, injury, pregnancy, heart condition, respiratory condition, recent surgery, illness, or any concern that may affect my ability to train safely.

I agree to inform Ryze Martial Arts & Fitness Academy of any medical condition, injury, limitation, or concern that may affect my participation.

In the event of an emergency, I authorize Ryze Martial Arts & Fitness Academy staff to contact emergency medical services if necessary. I understand that I am responsible for any medical expenses related to my care.

4. Personal Belongings

I understand that I am responsible for my own personal belongings, including clothing, bags, phones, wallets, money, shoes, training gear, water bottles, and any other items brought to the academy.

Ryze Martial Arts & Fitness Academy is not responsible for lost, stolen, damaged, or misplaced items.

5. Respect and Code of Conduct

All members are expected to show respect for:

The academy

Instructors and staff

Themselves

Training partners

Other students and families

Academy equipment and property

Members must follow instructor directions, train safely, use control with training partners, and maintain a respectful attitude.

Disrespectful behavior, unsafe training, bullying, harassment, threats, intimidation, intentional injury, stealing, destruction of property, or conduct that creates a safety concern will not be tolerated.

6. Martial Arts Use and Training Responsibility

I understand that martial arts training is provided for discipline, fitness, confidence, sport, self-defense, and personal growth.

I agree not to misuse martial arts techniques to bully, threaten, intimidate, or intentionally harm others inside or outside the academy.

I understand that unsafe training, uncontrolled sparring, intentional injury, or misuse of techniques may result in removal from class, suspension, or termination of membership.

7. Hygiene and Health Policy

I agree to maintain proper hygiene while training. This includes wearing clean training clothes, keeping fingernails and toenails trimmed, and not attending class with contagious illness, fever, open wounds, or skin conditions that may spread to others.

Ryze Martial Arts & Fitness Academy reserves the right to ask a member to sit out or leave class if there is a health, hygiene, or safety concern.

8. Weight Room and Sauna Use

I understand that the weight room and sauna may be available as part of my membership or academy access.

I agree to use all equipment safely and responsibly. I understand that I am responsible for knowing my own limits and asking for assistance when needed.

I understand that sauna use may involve risks such as dehydration, dizziness, overheating, or other health concerns. I agree to use the sauna responsibly, limit my time, hydrate properly, and stop use immediately if I feel unwell.

Ryze Martial Arts & Fitness Academy is not responsible for injuries or illness resulting from unsafe or improper use of the weight room, sauna, or academy equipment.

9. Media Release / Photo & Video Permission

I understand that Ryze Martial Arts & Fitness Academy may take photos or videos during classes, training, academy events, promotions, ceremonies, tournaments, or other academy-related activities.

Photos and videos may be used for safety, training, memories, marketing, social media, website use, flyers, and other academy-related promotional materials.

10. Academy Rules and Membership Termination

Ryze Martial Arts & Fitness Academy reserves the right to suspend or terminate membership if a member’s behavior creates a safety concern, repeatedly disrupts class, violates academy rules, misuses martial arts training, harasses or threatens others, damages property, or negatively affects the safety and experience of other members.

Membership cancellation, refund, and payment policies are handled according to the academy’s membership agreement and billing policies.

11. Member Agreement

By signing below, I confirm that I have read, understood, and agree to the terms of this waiver and agreement.

I understand the risks involved in martial arts, fitness training, weight room use, sauna use, and academy activities. I agree to take responsibility for my own safety, health, belongings, conduct, and participation.
