1. Acknowledgment of Risk
I understand that participation in the Ryze Martial Arts & Fitness Academy Summer Program may include Brazilian Jiu-Jitsu, martial arts training, fitness activities, games, drills, stretching, partner work, and other supervised activities.
I understand that these activities involve physical movement and contact. I understand there is a risk of injury, including but not limited to bruises, sprains, strains, falls, accidental contact, soreness, or more serious injury. I acknowledge that injuries may occur even when safety rules are followed and instructors are supervising.
By signing this agreement, I give permission for my child to participate in the Ryze Summer Program.
2. Release of Liability
I agree to release and hold harmless Ryze Martial Arts & Fitness Academy, its owners, instructors, coaches, staff, volunteers, representatives, and affiliates from liability for injuries, accidents, losses, or damages that may occur during participation in the summer program, except in cases of gross negligence or intentional misconduct.
I understand that it is my responsibility to inform Ryze Martial Arts & Fitness Academy of any medical conditions, allergies, injuries, medications, behavioral concerns, or limitations that may affect my child’s participation.
3. Medical Permission
In the event of an emergency, I authorize Ryze Martial Arts & Fitness Academy staff to contact emergency medical services if necessary.
I understand that staff will make reasonable efforts to contact the parent/guardian or emergency contact listed on this form.
I understand that the parent/guardian is responsible for any medical expenses related to the participant’s care.
4. Personal Belongings
Participants are responsible for their own personal belongings, including clothing, bags, water bottles, phones, money, shoes, training gear, snacks, and any other personal items brought to the academy.
Ryze Martial Arts & Fitness Academy is not responsible for lost, stolen, damaged, or misplaced items.
We recommend that participants do not bring valuable items to the summer program.
5. Pick-Up, Late Pick-Up, and Extended Care Policy
Participants must be picked up on time at the end of the scheduled program day.
The Summer Program ends at 3:00 PM. If a participant is also attending the evening kids class, families may request Extended Care from 3:00 PM to 5:30 PM for an additional $20.00 per day. Extended Care must be arranged and approved in advance.
I understand that late pick-ups affect staff scheduling and academy operations. If my child is picked up late without prior approval for Extended Care, I may be charged a late pick-up fee.
Repeated late pick-ups may affect my child’s continued enrollment in the summer program.
Late Pick-Up Fee: $20.00 after dismissal time, plus $1.00 for every minute past dismissal time.
6. Respect and Behavior Expectations
All participants are expected to show respect for:
The academy
The instructors and staff
Themselves
Their teammates
Other students and families
Academy equipment and property
Participants must follow instructor directions, use safe behavior, keep hands and feet to themselves unless participating in supervised training, and treat others with kindness and respect.
Participants are expected to help maintain a positive, safe, and respectful environment.
7. Bullying, Aggression, and Unsafe Behavior
Ryze Martial Arts & Fitness Academy does not tolerate bullying, harassment, threats, intentional harm, unsafe behavior, stealing, destruction of property, repeated disrespect, or behavior that puts others at risk.
If a participant is caught hurting others, bullying, threatening, intimidating, or acting in a way that creates a safety concern, the academy may use a strike system before considering suspension or termination of services.
Strike System
Strike 1: Verbal warning and parent/guardian notification.
Strike 2: Written warning and parent/guardian discussion or meeting.
Strike 3: Possible suspension or termination from the summer program.
Ryze Martial Arts & Fitness Academy reserves the right to immediately remove a participant from the program without completing all strikes if the behavior is severe, dangerous, intentional, or creates a safety concern for others.
8. Mandatory Reporting / Duty to Report Concerns
If Ryze Martial Arts & Fitness Academy staff observe or become aware of concerning behavior, suspected abuse, neglect, threats of harm, serious injury, unsafe conduct, or any issue that may require reporting, we are obligated to take appropriate action.
This may include notifying a parent/guardian, contacting emergency services, or reporting the concern to the proper authorities when required.
9. Media Release / Photo & Video Permission
I understand that Ryze Martial Arts & Fitness Academy may take photos or videos during the Summer Program for safety, training, memories, marketing, social media, website use, flyers, and other academy-related promotional materials.
I understand that Ryze Martial Arts & Fitness Academy will not intentionally post personal identifying information such as my child’s full name, address, phone number, school, or other private details without parent/guardian permission.
10. Program Termination
Ryze Martial Arts & Fitness Academy reserves the right to suspend or terminate a participant’s enrollment if the participant’s behavior creates a safety concern, repeatedly disrupts the program, violates academy rules, or negatively affects the experience and safety of other participants.
Refunds, if any, will be reviewed on a case-by-case basis.
11. Parent/Guardian Agreement
